Dr. Redekopp's patients will be asked to sign a roster form. This does not mean any change in Dr/patient relationship but when signed will allow Dr. Redekopp to become that patient's primary care physician. The advantages for patients are:
Patient access to health care is expanded beyond the regular business hours.
Patients are able to access healthcare with and through the same health care provider over time. This will allow the continuity of care received from Dr. Redekopp (working in partnerships with patients) to facilitate their access with other health-related services and health care providers when required.
Patients have access to all primary care members in the enrolling group, after hours clinics and the service of Telehealth Ontario # 1-866-797-0000 TTY : 1-866-797-0007
